Skill Level: Intermediate - You should be familiar with Photoshop's tools and interface and be comfortable working with the Channels and Layers palettes.

Written for Adobe Photoshop CS although everything in this effect can be accomplished all the way back to Photoshop 3. Maybe also possible with Photoshop Elements.
